Yes, if the birth took place in other states in Malaysia, the completed EFS application form and supporting documents must be submitted to the Headquarters of the National Registration Department, State of Sarawak within one year from the date of birth of the child. Dokumen-dokumen yang diperlukan adalah seperti berikut:
- Salinan MyKad ibu atau bapa berstatus ‘K’ yang telah disahkan oleh pegawai JPN; dan
- Copy of baby's birth certificate verified by JPN officer.
Ya, bagi kelahiran di luar negara, kelahiran perlu didaftar di Pejabat Kedutaan Malaysia bagi mengesahkan ibu bapa memilih kewarganegaraan Malaysia bagi anak tersebut sebelum menghantar permohonan ke Jabatan Pendaftaran Negara, Negeri Sarawak dalam tempoh 1 tahun. Sila lampirkan:
- Full completion of the EFS application form;
- Salinan MyKAD ibu atau bapa berstatus ‘K’; dan
- Salinan sijil lahir bayi atau Borang K.
Sekurang-kurangnya 90 hari bekerja tertakluk kepada kelengkapan dokumen yang dihantar kepada JPN/Bank Islam/KPWK.
