1. Salinan MyKad ibu atau bapa berstatus ‘K’ yang telah disahkan oleh pegawai JPN; dan
2. Copy of baby's birth certificate verified by JPN officer.

Ya, bagi kelahiran di luar negara, kelahiran perlu didaftar di Pejabat Kedutaan Malaysia bagi mengesahkan ibu bapa memilih kewarganegaraan Malaysia bagi anak tersebut sebelum menghantar permohonan ke Jabatan Pendaftaran Negara, Negeri Sarawak dalam tempoh 1 tahun. Sila lampirkan:

1. Full completion of the EFS application form;
2. Salinan MyKAD ibu atau bapa berstatus ‘K’; dan
3. Salinan sijil lahir bayi atau Borang K.

Yes. If the child dies before reaching the age of 18, the next of kin can withdraw from the EFS for the purpose of funeral management by submitting the original death certificate and MyKad to the nearest Bank Islam branch.
